It shows up on MRI reports, it is the focus of most rehabilitation programs, and it is the area where athletes feel symptoms most clearly.

So the conclusion becomes automatic. If the shoulder hurts, the rotator cuff must be the problem.

That assumption drives almost everything that follows. Band external rotations, light dumbbell work, T’s and Y’s, and general strengthening exercises become the default solution. For a short period, some athletes feel better. But when they return to pressing, throwing, or higher training volumes, the pain comes back.

That is usually the moment where things stop making sense. The athlete is stronger, more consistent, and doing everything they were told, yet the problem persists.

When the arm moves overhead, all of these components must work together in a precise sequence. The humerus does not simply elevate. The scapula must rotate and reposition, the clavicle must move, and the thoracic spine must extend enough to allow the system to function.

This is often described as scapulohumeral rhythm, but most explanations oversimplify it. In reality, this is a timing and coordination problem under load.

This becomes especially important in overhead athletes, where speed, load, and repetition magnify even small inefficiencies. If that coordination is clean, the shoulder feels smooth and efficient. If it is not, stress begins to accumulate in predictable areas. This is why two athletes can perform the same movement and only one has pain. The issue is not the movement itself. It is how the system organizes that movement.

What the Scapula Must Do During Overhead Motion

During arm elevation, the scapula must upwardly rotate, posteriorly tilt, and externally rotate. These movements allow the glenoid to remain aligned with the humeral head so the joint stays centered throughout the motion.

When this happens correctly, the rotator cuff can function efficiently, and load is distributed across the system. When it does not, the mechanics shift. Upward rotation that is insufficient means the socket is no longer positioned well beneath the humeral head. Limited posterior tilt changes how the acromion relates to the rotator cuff. Poor external rotation of the scapula alters the orientation of the joint.

The scapula may elevate too early rather than rotate effectively. The humerus may move before the scapula is ready. These are not strength deficits in the traditional sense. When these coordination issues are repeated during training, the system begins to overload. The rotator cuff and surrounding tissues absorb stress they were never meant to handle in isolation. However, the issue is rarely just whether these muscles are strong. The issue is whether they are contributing at the right time and within the correct movement pattern.

An athlete can have strong traps and still have poor scapular motion. They can feel their serratus working in an exercise but not use it during pressing or throwing. They can complete all of their rehab exercises perfectly and still move the same way when it matters.

At the same time, other muscles can begin to dominate in ways that disrupt scapular mechanics. The pectoralis minor can pull the scapula into anterior tilt. The rhomboids can limit upward rotation. The latissimus dorsi can influence both shoulder motion and trunk position, especially in athletes who rely on extension strategies. This creates a system where the scapula is no longer providing an efficient base for the shoulder.

Can Thoracic Stiffness Cause Shoulder Pain?

One of the most underappreciated drivers of shoulder dysfunction is the environment the scapula sits on. The scapula rests on the rib cage. If the rib cage is poorly positioned or the thoracic spine is stiff, the scapula cannot move effectively.

Athletes who live in excessive lumbar extension and rib flare often struggle to achieve proper scapular posterior tilt and upward rotation. The shoulder is then forced to compensate.

Pressing, throwing, lifting, and sport-specific tasks must reflect the new mechanics. If the pattern does not carry over into training, it does not matter how good the exercises look in isolation.
